Case Manager Supervisor - BILINGUAL (Spanish & English)

  • Responsibilities:
  • Support the Programs Director in ensuring the timely flow of case management activities to meet programs goals and objectives;
  • Oversee the integration of trauma-informed services.
  • Supports Case Managers in assessing participant's immediate needs, legal, family and personal circumstances, and engagement needs to develop safety and Individual Family Plans;
  • Use motivational interviewing and supervise staff in use of motivational interviewing to engage and retain participants in services;
  • Provide regular supervision to case management staff to ensure they are providing services according to agency's policies and procedures,
  • Allocate time to direct participant services and programs, including direct work in the community and with staff as needed;
  • Complete assessments, progress notes and significant program data as needed.
  • Along with the Programs Director lead case management team meetings.
  • Supervise electronic chart-keeping activities in accordance with confidentiality regulations;
  • Provide in-service training and promote staff development to ensure case management staff have core competencies;
  • Provide culturally competent services in accordance with agency's policies and practices;
  • Help ensure that programs achieve expected performance milestones in accordance with contracts and expectations of funding sources;
  • Supervise staff and support t e alignment of individual staff performance targets towards overall program goals and objectives;

Minimum Qualifications

  • Master's Degree in Social Work (Permanent Professional license required).
  • Minimum 5 years of professional experience in a Clinical or Behavioral field. Case Management experience is a plus.
  • Supervision experience with strong leadership skills.
  • Experience conducting comprehensive assessments and or treatment plans.
  • Ability to work under pressure with minimum supervision.
  • Strong verbal and written communication skills.
  • Knowledge in community based resources.
  • Computer knowledge in Microsoft Office: Word, Excel, Power Point, Zoom etc. to
  • Fully Bilingual- Spanish and English (Oral and Written)
  • Ability to pay attention to detail, manage time effectively and being able to multitask in a fast-paced environment are required
